Physical Examination

Students registering for Athletics must have a current physical examination form downloaded for the School Nurses review before the student is authorized to participate in practice or tryouts. Physicals are valid for 12 months from the date of the exam. Download the physical form by clicking the Athletic Team Registration icon, completing a family account, and following the download instructions.

Physicals are valid for 12 Months from the date of the last exam.
